What is Hysterectomy?

Hysterectomy can be called as a surgical removal of woman’s uterus considering some amount of dreaded disease in the same. With this surgery, the women end up getting the menstruation periods and in other words loses the ability to become pregnant. Thus depending upon any reason for the surgery, the hysterectomy can involve the surgical removal of the woman along with tissues and other organs in the surrounding like ovaries and fallopian tubes provided these are also infected by the same.

Why Is a Hysterectomy Performed?

When it comes to carrying out the procedure of hysterectomy, there can be a number of reasons to do so, some of these are as under:
  • Abnormal vaginal bleeding, which is not regulated by any other treatment options
  • Severe kind of endometriosis
  • The presence of uterine fibroids (benign ones), which are found in bigger size and that happen to be painful or are seen carrying out the bleeding.
  • Increased amount of pelvic pain linked to uterus that cannot be controlled by any other treatment
  • Uterine prolapse that can lead to issues like urinary incontinence or bowel movement difficulty
  • Cervical or uterine cancer ailments leading to cancer for cancer prevention

What Are the Types of Hysterectomy?

When it comes to the types of hysterectomy, there are several of the options found in the medical field, the common one can be jotted down as under:

Partial Hysterectomy: In this procedure, one can find the ovaries or the cervix that are left whole. These procedures too can be carried out abdominally, laparoscopically or vaginally. Regrettably, whole lot of women are seen either not much aware about the same or do not know all such options really existed. There are number of doctors who all are seen taking out the cervix automatically like a precaution against the cervical cancer.

Total Hysterectomy: As the name indicates, this surgery deals with removing the entire uterus of the woman along with cervix too are removed. The surgery can be carried out abdominally with a small incision and thus can be carried along the bikini line. In more emergent situations one can find vertical type of incision that has to be made over the abdominal wall found over the belly button to the pubis. This kind of surgery is needed with more downtime and the healing time too. The procedure is carried out via the vagina with no bigger incision made over the belly.

Hysterectomy and Salpingo-Oophorectomy: Both the procedures deal with removal of organs like fallopian tube and ovary, which together called the Salpingo-Oophorectomy. A unilateral salpingo- oophorectomy is relevant for women whom an ovary is unable to be preserved that can include the cases of ruptured ectopic pregnancy having the incapability to get hemostasis without actually the removal of ovary, tube and adnexal torsion wherein the tube and ovary are necrotic.

Advance Hysterectomy surgery- The surgery remains the same that deals with removing of the organ called uterus from the woman’s body, however, the method used for the same has turned more advanced. 8 Common Myths About Ovarian Cancer

March is “Ovarian Cancer Awareness Month” and here we bring you some of the common myths about the ovarian cancer.
Ovarian Cancer Awareness Month March 2016

  1. It is a “Silent Killer”: Often, ovarian cancer is referred as a silent killer since it has no symptoms. However, it is not true. The key symptoms of ovarian cancer are persistent bloating or increased stomach size, persistent stomach pain, urge to pee more frequently and difficulty eating or feeling full quickly. These symptoms can happen to us sometimes, but if the symptoms are unusual and they don’t go away then you should speak with your GP.
  2. It doesn’t affect young women: It is not well-known as some other female cancer and there are a lot of myths surrounding this disease. Over 7000 women are diagnosed and about 4300 deaths are reported for ovarian cancer in the UK. Ovarian cancer is the fifth most common female cancer and often it is associated with women over the age of 50. Though it is true that about 80% of the women over 50 plus are diagnosed with ovarian cancer, it leaves one in five who are younger.
  3. It is the same as cervical cancer: There are lots of different gynaecological cancers so, its better to be aware of each one of them. The five main gynaecological cancers are cervical, ovarian, vaginal, uterine and valvular cancer and each of these have different symptoms. If you suspect anything is wrong, get to know about your body by speaking with your doctor.
  4. Cervical smears will detect it: There is no routine test for detecting ovarian cancer, so a cervical smear won’t detect it. The best way to detect this disease is by being symptoms-aware.
    Ovarian Cancer Symptoms
  5. BRCA means only breast cancer: Often a BRCA gene mutation is discussed in the media alongside an increased risk of getting breast cancer. However, if you have a BRCA gene mutation, then you have a 35-60% increased chance of developing ovarian cancer too. BRCA gene mutations can be hereditary.
  6. You can do nothing to reduce its risk: Taking contraceptive pills could reduce your risk of ovarian cancer. A few lifestyle factors such as obesity, smoking, giving birth to your first child after 30’s, not breastfeeding, not having any children and using HRT can increase your risk of developing ovarian cancer. Other contributing factors include, getting older, having endometriosis and a family history of breast and ovarian cancer.
  7. Women who have done as hysterectomy can’t get ovarian cancer: There are different types of hysterectomy. In most cases, the womb and the cervix are removed and in certain cases, the Fallopian tubes and ovaries too, are removed. Although removal of the ovaries reduces the risk by 95%, but still there is a 5% risk of developing ovarian cancer cells. You are also at risk of developing peritoneal cancer, which is similar to ovarian cancer.
  8. HPV vaccine helps: Sadly, HPV (Human Papilloma Virus) vaccine does nothing in the way to prevent the ovarian cancer. Though this vaccine protects against cervical cancer, but currently there is no equivalent vaccine available to protect the women from developing ovarian cancer.

9 Safe Routines to Get You Move Again After a Hysterectomy

Hysterectomy is a surgical procedure involving the removal of a woman’s uterus. Hysterectomy is the second most common surgery among women of reproductive age with about 600,000 procedures being performed each year. The primary reason for hysterectomy include endometriosis, pelvic pain, shifting of the uterus and cancer of the uterus, ovaries or cervix and uterine fibroids.

There are three different degrees of hysterectomy; total, partial and radical. Your doctor will make a recommendation based on the patient’s medical condition and history. Recovery time will vary depending on the type of surgery performed.

Types of Hysterectomy:
  1. Abdominal Hysterectomy
  2. Vaginal or Laparoscopic-Assisted Vaginal Hysterectomy (LAVH)
  3. Laparoscopic Supracervical Hysterectomy (LSH)

As per the American College of Obstetricians and Gynecologists, the patients undergoing this procedure should get lots of rests, take short walks and refrain from lifting the heavy objects.

Exercising After Undergoing a Hysterecomy:

Exercises After a Hysterectomy

Post-hysterectomy exercise is one of the key factors in the healing process by promoting healthy sleep, assisting in weight loss, relieving muscle tension and preventing a range of diseases, from various types of cancer to diabetes and Alzheimer’s. Women who were physically fit before undergoing this procedure are more likely to have an easier recovery and will be able to quickly return back to their routine physical activities.

The safe routines to get you moving again posy hysterectomy include the following:

Safe Routines After a Hysterectomy

  1. Walking: You can start walking on your doctor’s approval. Take short, frequent walks; this will help to speed up the healing and recovery process and will reduce the risk of blood clots.
  2. Breathing Exercises: It is quite common to experience difficulty breathing in the hours and days following a hysterectomy. Take long, slow breaths, completely filling the lungs, belly and rib cage before slowly exhaling. Targeted breathing exercises will help you return to a normal, comfortable respiratory pattern.
  3. Stretching: Gentle stretches will help relieve muscle tension and promote healthy blood circulation. Stretch your upper back and shoulders while lying on the bed or on the floor. Bend your knees and reach your arms up over the head, lightly press them against the floor or the bed and then release. Repeat this procedure. As you get stronger and more mobile, you can add more advanced stretches to this routine.
  4. Pelvic Tilts: It helps to strengthen the pelvic floor. Lie on your back with your knees bent and support your head with a pillow. Lift your bottom up into a bridge position while contracting your stomach muscles. Keep your middle back flush against the floor. Maintain this position for a few seconds, release and repeat this procedure.
  5. Kegels: Post hysterectomy, your pelvic floor may be weakened and this can cause loss of the bladder control, shifting of pelvic organs and other problems. They can be performed in any position and at any time. Simply, squeeze the muscle you would normally use to stop the flow of urine, release and repeat. Aim for high repetitions of varying lengths.
  6. Head Sit-Ups: If you are not ready for full-crunches, then start with a head sit-up. Lie on your back, bend your knees and cross your arms over the belly. Cinch the stomach muscles with your hands, slowly raise your head off the floor and bring your chin to your chest. After a few seconds, release and repeat.
  7. Stomach “Vacuums”: This move helps strengthen the stomach muscles safely. Start from your hands and knees, inhale and then tighten your tummy as you slowly exhale. After a few seconds, release and repeat it.
  8. Lower Back Exercises: They help to strengthen the core while improving the posture and stability, thereby aid in healing and recovery. It also helps to alleviate and prevent the back pain post hysterectomy.
  9. Other Abdominal Exercises: It’s normal to experience bloating or loss of abdominal strength after a hysterectomy. Although diet and cardio help in slimming down the midsection, targeted core exercises helps to strengthen those muscles. Ensure to give special attention to the transverses abdominis (TA) muscles by performing, planks, Pilates moves, bridges and forward ball rolls.
Wrap-up:

Always check with your doctor before starting or resuming an exercise routine after undergoing a hysterectomy surgery. Walk and stretch as soon as your doctor gives you the green signal. Do not lift anything heavier than five pounds for the complete recovery period. Engage yourself in abdominal exercises only after the full recovery period. Even if you feel good, do not overdo it since you can strain the incision or cause an internal injury.

Based on the cause of hysterectomy, the uterus should be removing completely or only a part of it should be removed can be decided by the surgeon. An individual might need to undergo hysterectomy due to the mentioned reasons:

Infection in the pelvis

Pelvic region infection or the PID (Pelvic Inflammatory disease) is an infection of the reproductive organ of the female. When you are considering STD in women (i.e. sexually transmitted disease), it is one of the most dreaded reason. It can further cause the irreversible damage to the ovaries, the uterus, fallopian tubes and other reproductive system parts of a women body, this is what makes it a primary reason behind of women infertility, thus need to be undergo treatment.

Fibrosis on the wall of the uterus

It is a condition in which the inside lining of the uterus breaks due to the muscle wall of the uterus. This fibrosis on the wall further cause lower abdominal pressure, menstrual cramps, and can result in heavy menstrual periods. This condition might affect only a single spot of can affect the whole uterus of women. Though it is non-life threatening condition, but one with this condition can have negative impact on quality of life.

Endometriosis

It is generally a chronic disorder which long-lasts. The line of the uterus cells/tissue is also start developing external side of the uterus, when an individual is experiencing endometriosis. The implants (clumps of tissues) may have spread on the external wall of uterus, ovaries or fallopian tubes, intestine and/or other organs. In some cases it might spread to regions beyond belly. But if the clumps of tissues grows in a sensitive region it might cause constant pain and pain during some activities, which make it worth undergo for treatment.

Uterine fibroid

It is a kind of tissue which develops outside or inside a women’s womb (uterus) and is non-cancerous in nature. It develops from normal cells of uterus muscle which start developing abnormally. It forms a benign tumor as the cell starts growing. These are extremely common, these are the benign lumps grow on the women uterus. Hysterectomy is one of the treatment options.

Hysterectomy: Know Your Surgical Options

Overview
A hysterectomy surgery is nothing but a procedure of uterus removal, which also deals with removing organs like ovaries and cervix. Though this procedure is often used to remove the fibroids over the uterus, but at times it can be even used to remove the above said organs as well.  In the year 2013, more than 1,500,000 were performed in the west for the benign conditions. Such types of rates are considered to be the highest in the world. The latest figure indicates that out of 10,000 women 300 undergo for this surgery. Now, let’s us dig deep into this procedure and find out the next as under:

Minimally Invasive Hysterectomy Options

When it comes to Minimally Invasive Hysterectomy Options, the following are the choices, let’s check them out:
  • Vaginal Hysterectomy: This surgery is carried out making small incision over the top side of the vagina wherein the surgery is seen accessing the uterus via cutting the blood vessels, ligaments and fallopian tubes. The surgeon then cuts the uterus, which is then removed via the vagina. This procedure has a number of benefits, which include no scars or pain after the surgery, while the disadvantage is that it can be difficult to carry out this procedure for many reasons.

  • Laparoscopic supracervical hysterectomy: This surgery is performed by making a couple of incisions over the lower abdomen area, which passes through the vertical way inside the bone via the belly button along the bikini line. There are couple of benefits of this surgery, which include removing the uterus without any internal scarring even when the fibroids are of big size. The recovery period is not much as compared to the other surgical options.

  • Laparoscopically assisted vaginal hysterectomy: The name itself indicates that it is a surgery with a laparoscopic approach, which helps in removing the uterus, or fallopian tubes via the vagina. This procedure helps in getting a right view of the uterus, which helps in removing the fibroid easily seeing over the screen. In case if the laparoscope is not inserted inside the vagina then the surgeon is seen making smaller incisions over the abdomen area to do the needful.

  • Robotic-assisted hysterectomy: This procedure is among the latest one to add for the women needing hysterectomy. In this procedure, you enjoy a wide range of benefits like less pain, faster recovery, lesser hospital stay, and many more. This surgery is carried out by making special instrument based on robotic system over the area near (lower abdomen) to uterus. Using a camera the surgeon gets a much better image over the screen, which helps the doctor to do the needful.
When a Traditional Hysterectomy Is Needed?
There are many reasons for performing this procedure, however, the following are the most common ones, let’s check them out:
  • For having the presence of Uterine Fibroid
  • For the abnormal uterine bleeding or the vaginal bleeding
  • Cervical dysplasia
  • For having Endometriosis & Uterine Prolapse like  getting pelvic relaxation
  • For Cancer
As per reports, only 10 percent of this procedure is carried out to remove cancer, while the rest 90 percent is carried out for extracting the non cancerous tumours etc.

Which Hysterectomy Surgery Option Is Right for You?

There are various types of Hysterectomy surgery, which you may require as per the conditions and issues being based by one. Some of these procedures are as under:
  • Radical Hysterectomy: This surgery is need when you need to remove the cervix, ovaries and uterus
  • Total Hysterectomy: This surgery is required when you are supposed to get rid of the cervix and uterus
  • Subtotal or partial hysterectomy: This surgery can help you in removing the upper portion of the uterus, which then leaves behind ovaries and cervix behind in a perfect condition.

Types of Laparoscopy Surgery

Hysterectomy:
Hysterectomy is a surgery to remove the uterus. It helps prevent future pregnancy as well as reduces fibroid-related bleeding and pressure symptoms.

Laparoscopic removal of endometriosis:
Our medical experience tells us that much better outcomes are got when endometriosis is excised (remove) , rather than cauterized or lasered, whenever there is the slightest hint that the disease goes deeper than the most superficial layers of pelvic tissue.

Laparoscopic removal of ovarian cysts:
Ovarian cysts up to 10 cm (3.5 inches) in diameter are routinely removed laparoscopically in our division.

Laparoscopic removal of adhesions (scar tissue):
When careful evaluation has proven that adhesions may play a role in a person's abdominal or pelvic pain , we sometimes recommend laparoscopic surgery to try to reduce the amount of adhesions present.

Laparoscopic removal of a tube and ovary:

When an ovary is too involved with a disease process to salvage, it is generally easy to cut it away using laparoscopic techniques.

Myomectomy:
Myomectomy is the surgical removal of uterine fibroids without the removal of the uterus.

Laparoscopic uterine suspension:
In about 15-20% of women, the top end of the uterus leans back toward the backbone, instead of leaning forward , toward the bladder.

Laparoscopic bladder support surgery:
Many different surgeries have been produced to deal with "stress incontinence," or loss of urine during coughing, laughing, or any other physical activity.

Endometrial ablation (for heavy periods):
There are now about 8 different approved techniques for applying some form of energy to the lining of the uterus (the endometrium) in order to reduce the amount of menstrual flow for a person who has regular, but quite heavy, periods.
